Seeing a mother hold her newborn baby for the first time

Okay so this one isn’t really “simple” to come across, but when you do, it’s one of the most touching moments you’ll see. There’s something in the mum’s (and dad’s) smile that cannot be described by words. Definitely one of the (few) perks of OBGYN run. Man I can’t wait to have kids some day (I’ll regret saying this in the future).

Judgement of Solomon

The Bible tells many stories of a King Solomon, son of David. King Solomon is most famous for his wisdom, of which there are many accounts of in the Bible. The following is an example of the wisdom of Solomon.

There once lived two women living under the same roof who both gave birth to a son at similar times. One of the mothers accidentally smothered her own son while sleeping, and decided to switch the two infants, claiming the living one to be her own. The other woman instantly noticed that the dead baby was not hers and confronted the culprit, asking for her baby back. She refused, leading to a very heated argument that ultimately ended up in the court of King Solomon. The two women pleaded him to make the decision of who the real mother was. After much deliberation, King Solomon called for a sword to be brought before him. He stated that since both women were claiming the boy to be their own, there was only one solution: to split the baby in two and give each person a half of the baby. The lying woman, in bitter jealousy, urged King Solomon to cut the baby. She thought that if she could not have the baby, then no one shall. The true mother, mortified by what King Solomon planned on doing, pleaded him to just give the baby to the other women and not to kill the baby. The king then judged that she must be the true mother and gave the baby back to her, while punishing the other for her sins.

The story shows how the wisdom of King Solomon led to justice and reuniting the mother and baby by method of creating a fake situation that would instantly distinguish the actual mother from the liar. The expression “splitting the baby” is still used in legal professions to describe the act of coming to a simple compromise between two parties.

The intended moral of the story is probably to teach people that wisdom can defeat even the greatest of challenges. But perhaps the real moral of the story is: if you are insane enough to steal a baby, at least have enough acting skills to follow through with it instead of telling someone to kill the baby.

Caesarean Section

Most animals give birth through a female’s vagina. Of course humans are the same when it comes to natural birth, but nowadays, it is not uncommon to find women wanting a caesarean instead of the traditional method. A caesarean (also called C-section) is a surgical procedure where the fetus is taken out by cutting through the lower abdomen into the uterus. The history of caesareans is quite dark. Back in the old days when medicine was not advanced, caesareans were mostly used to rescue fetuses from mothers who had died during childbirth. The first record of a successful caesarean where the mother survived dates back to the 1500s. Many people believe the word “caesarean” came from the Roman emperor Julius Caesar, who was allegedly born via a caesarean. However, it was rare for caesareans to be performed in Roman times and even if they were, the mothers almost certainly died in the process. Given that his mother was alive and healthy well into his adulthood, it is highly unlikely that Caesar was born by caesarean (there are no concrete records of it either).

There is much debate to whether a caesarean is better or worse than natural birth (except in emergency situations where a caesarean is required). According to research (in cases without known risks to the fetus), the mortality rate is definitely higher in babies born by caesarean compared to those born naturally. This is most likely due to a caesarean bypassing some of the physiological changes that occur during vaginal birth.

Another debate is about the use of general versus regional anaesthesia (spinal block) when doing a caesarean. A fascinating fact about childbirth is that when a baby is born, it cries to expand its lungs but then quietens down for about an hour (unless it is in pain or there is some stimuli). This is possibly a mechanism to allow bonding between the mother and baby. New mothers often remember the moments following the birth of the child as extremely emotional and blissful. Contrastingly, mothers who are under general anaesthesia and not awake when their child is born bond less with the baby initially (some mothers do not even recognise the baby as their own). Thus, unless it is an emergency caesarean, a spinal block (which allows the mother to be awake and painless) is preferable over general anaesthesia.

Lastly, it is common tradition to cut the umbilical cord straight after the child is born. But is this okay? When the fetus is in the uterus, it shares its cardiovascular system with the placenta. The umbilical cord connects the two and carries blood to and fro. At any given point, the placenta contains 30~50% of the fetal blood. If the umbilical cord is suddenly cut, the fetus essentially loses this blood, being born in a state of low blood volume. If you look at the umbilical cord, you can see that it is about 1m in length, which is enough for the baby to be put next to the mother’s breasts for breastfeeding and bonding. Perhaps we are cutting the cord too soon, not letting the blood flow back from the placenta to the fetus.

If you think about it, humanity has been giving birth without too many problems to survive generation after generation for 200,000 years (otherwise we would not exist). Although the mortality rate was high, Mother Nature has optimised childbirth over time through evolution. Ergo, it is possible that modern medicine is intervening too much in a natural process. We must always consider whether medical advances are helpful or harmful to us.

Mother Horse

Female horses (mares) exhibit a very strange behaviour after they mate with a male. A mare will mate with every other male in the stable, almost immediately after mating with the first one. This strange post-coital promiscuity does not appear to make any sense from an evolutionary perspective, as one would think that pregnancy would lead to a decrease in libido so the mother can focus on caring for her embryo. However, this promiscuity is part of a strategy that helps protect the baby horses.

It is common behaviour in the animal kingdom for a male to kill a female’s young so that he can mate with her and produce babies with his own genetic material. This also happens in horses, where stallions engage in infanticide, kicking a foal to death if it is not his. For a mother horse, this is not only heartbreaking, but also a tremendous waste of the energy she put into pregnancy. So to protect her offspring and conserve energy, she ensures that no male knows who the father of her baby is. It has been shown that males who mated with a female who gave birth will not attack the foal, as there is a chance that it is his. Thus, a mother horse guarantees the protection of her child by prostituting herself to all of the males around.

Although the story of a mother willing to sacrifice everything, even her dignity, for the safety of her offspring may be inspiring, it has also been observed that if a mare cannot mate with all of the males in the stable, she will instead abort the pregnancy. For example, if a new horse is brought into the herd after the mare becomes pregnant, the mare senses the danger to her eventual foal and proceeds to abort it. In a study involving zebras, it was found that bringing in a new male made the foal’s chances of survival fall to less than 5 percent.

Han Suk-Bong

Han Suk-Bong is a famous writer from the Joseon Dynasty (Korea during 14th to 19th century), who was praised as “the master writer of the East” even in the Ming Dynasty (China during 14th to 17th century). His writing and calligraphy were partly thanks to his inborn talent, but also because of his intense training and practise throughout his life. There is a famous story regarding his training.

Han Suk-Bong practised calligraphy since a young age by himself, practising every day. The villagers all praised his talent and his mother sent him to a famous temple to study. After four years of studying, Han missed his mother so much that he sneaked out during the night and returned home. When he told his mother that he there was nothing more for him to learn, she told him to turn the lights off and said: “I will slice rice cakes while you write, then we will compare our skills”. After the two silently did their best work in the darkness, they turned the light on and it was evident that Han’s letters were all crooked and unsightly while his mother’s rice cakes were perfectly sliced in even thickness. Han deeply repented his arrogance and realised there was so much more to learn. His mother told him off and told him not to set foot in the house again until he could write perfectly even with his eyes closed, just as she could slice rice cakes perfectly. Thanks to his mother’s passion for his education, Han became one of the most well-known masters of calligraphy and literature in the Far East.

The best type of parent is one who identifies a child’s natural talents early on and helps them develop those skills. If the child becomes lost, loses their way or fall into the pit of arrogance believing they are the best, it is the parent’s duty to correct them. The moment you believe that there is nothing more to learn, you become a failure.

How To Calm A Baby

One of the hardest things about being a new parent is how to quell the cries of a newborn. Since crying is their only way of communicating needs, babies cry constantly and this can be extremely distressing for the parents. 
Of course the best way is to deal with what the baby needs, such as food or changing their diaper, but the cause is not always clear. Most parents use the traditional method of picking the baby up and slowly patting or rubbing the baby’s back. By patting at a similar beat to the mother’s heartbeat, the baby feels at ease as it reminds it of the relaxing state within the womb.

There are some other less conventional methods that have been shown to work. For example, turning the vacuum cleaner on or any other white noise (such as the washing machine, rustling a plastic bag or even gargling water in your mouth) has an instant effect of stopping the baby’s cries (NB: usually only works on infants below 3 months of age). The reason being, these noises are at a similar frequency to the noises the baby hears in the womb, such as the mother’s guts moving, blood flowing through the vessels and sounds from the outsides being transmitted through the mother. As the brain remembers such relaxing states for the fist few months, these stimuli induce a relaxing response and calm the baby.
Similarly, turning on rock & roll music (not too loudly) calms the baby in the first few months as they are unable to recognise the words but can still feel the vibration from the rhythm, which again reminds the baby of the womb.

The same principle can be used to simulate other features of womb life. Rocking the baby slowly simulates the sensation when the mother is walking, wrapping the baby in a blanket simulates the warmth and cosy nature of the womb and giving the baby something to suck on like a pacifier induces the powerful sucking reflex which calms the baby.
As a last resort, touching the inside of the ear canal with your little finger causes the baby to become confused about the strange sensation, distracting the baby. Although this method is effective up to 24 months, it is not recommended as it can lead to an ear infection.

On a similar note, when a baby or young child is stubbornly holding on to something, the best method to get it back is for the mother to pretend that they are crying. The child, empathising with the mother and not wanting her to be sad, yields the object in their hand to make her happy. But this method may not work after a while when the child realises you are tricking them.

Security Blanket

In the Peanuts comic strip, the character Linus van Pelt is always seen with his trustworthy security blanket. What is it about a simple blanket that lets certain children feel so safe around it, and why do they become so agitated when it is taken away from them?

Psychologically speaking, the idea of attachment plays a heavy role in the child’s obsession with their security blanket (or any other comfort object, such as a teddy bear).
An infant’s perception of the world is very limited and it cannot understand the concept of “self” until it develops further. In fact, it is theorised that an infant believes that whatever it wishes, the mother (still considered by the infant as “self”) will bring it to it, thus creating an illusion of omnipotence.
When the realisation that there is something other than “me”, the baby becomes frightened. It suddenly understands that the mother and it are not one, but two separate beings. At this point, it loses the sense of omnipotence and realises it is dependent on others, creating a loss of independence.

Losing its independence and a large portion of itself (the mother), the baby becomes confused and anxious, a phenomenon paediatricians call infant’s lament. The baby tries to comfort itself by attaching itself to its first “not-me” possession – such as a blanket or teddy bear, also called a transitional object. This then allows it to be separate from the mother for periods of time. The transitional object is a reminder to the baby that it still has some control over life and some independence, which gives it comfort and allows the baby to sleep better at night (literally).
Thus, the security blanket is aptly named, as it provides the baby with the confidence and security to adapt to the new world, allowing the baby to grow and develop into a social being.

When the baby develops into a child, it develops its own sense of self-confidence so that it can detach from the transitional object. However, some children never detach themselves and the security blanket persists for a longer time. Unfortunately, this is often found socially unacceptable and seen as a sign of weakness.
Interestingly, studies show that these children are often more independent than other children, due to their ability to be less dependent on their parents. The security blanket never criticises or doubts the child’s abilities, therefore gives the child a source of infinite confidence.

Another research by Lucy van Pelt shows that removal of the security blanket from a child results in withdrawal symptoms such as fear, panic, perspiration, glazed eyes and unconsciousness within 50 seconds.

From Cell To Birth: Childbirth

The onset of labour signals that the birth of the child is imminent. Strong contractions of the uterus under the influence of oxytocin cause intense pain for the mother. This escalates as oxytocin creates a vicious cycle for more and more powerful contractions. Other signs include a bloody show, where blood and mucus leaks out, and the water breaking, where amniotic fluid bursts out as the membranes rupture.
When the cervix has fully dilated to 10cm wide, the baby is ready to come out, and this is the end of the first stage. It may take over 24 hours for this stage to be over – all the while the mother is in extreme pain as her body transforms for the process.

The second stage concerns the delivery of the baby, and follows a precise series of steps.

  1. The baby, oriented head-down, facing either right or left, leans against the pelvis.
  2. The baby begins to descend, passing through the pelvis.
  3. It flexes its head as it comes through the cervix, turning towards the mother’s back as it does so.
  4. As the head crowns, the baby turns back to its side-facing position so the shoulders can come out.
  5. The rest of the body comes out with the shoulder. Forceps may be used to help.

The passage of such a large head and body is only possible through the massive dilation of the cervix, the stretch of the vagina, and the soft, malleable skull of the baby (a newborn’s head often looks conical and alien-shaped). Quite often, the opening of the vagina is not wide enough, and the opening may be cut open more to ensure it does not rip.

After the baby is safely delivered, the umbilical cord and the placenta are also delivered, thus ending the third stage (afterbirth). The cord is clamped and cut, and the baby is then thoroughly checked to ensure it is healthy and has no developmental issues.

Although in the past childbirth was extremely risky and often led to the death of the mother and/or baby in the process, nowadays protocols and health professionals ensure that both survive the procedure, thus ending the incredible 40-week journey until the birth of a new life. Only after understanding the arduous steps and the risks a mother takes to give birth to her child can one truly appreciate the value of life.

From Cell To Birth: Growth

After implantation, the embryo quickly grows from a ball of cells into what will be a fully-formed baby. However, it first needs a way to feed: the placenta.
It is an organ that actively takes nutrients and oxygen from the mother’s blood, exchanging it for the embryo’s waste products. It is extremely effective in keeping the fetus alive and protects it from infections or the mother’s immune system.
The blood is carried by the umbilical cord, which plugs into the belly button. This cord is the lifeline throughout term, and disrupting the blood supply will lead to permanent brain damage or even death.

In the first 10 weeks, the blastocyst develops into a very primitive disk-like object that shares no resemblance to a person. It keeps growing and differentiating at a rapid rate (almost doubling in size per week) until it forms an embryo that is more familiar, roughly about week 6. Interestingly, a human embryo looks almost identical to embryos of rabbits, chickens, turtles and fish, showing how all animals shared a common ancestor in the course of evolution. At this stage, the embryo has features such as gills, a tail and a fish-like appearance.

After 10 weeks, the embryo has grown to about 5~8cm (almost 10~20 times the size at week 6), and is now called a fetus. It begins to properly grow organs, and resembles a miniature baby with primitive features.
It continues to grow for the next 30 weeks, continuously relying on the mother for nutrition and life support.

Many different factors contribute to premature birth and IUGR (intrauterine growth restriction), which leads to the birth of a small baby. This may result in less developed organs (especially the lungs) and may affect the health of the newborn throughout its life. There are also many poisons known to harm the development of the embryo/fetus, such as alcohol, nicotine, cocaine, heroin and much more. These should be avoided from a few weeks before conception onwards (even after birth while breastfeeding).

By about 38 weeks, the lungs (the last organs to fully mature) are ready and the fetus is upside down. It is ready to leave the womb, and thus sends a signal to the mother, known as labour. This is when the arduous process of childbirth begins.
